PARTICIPANT SAFETY PICKUP FORM

In an effort to help keep our participants safe, everyone picking up a child must complete this form and show both photo ID & the confirmation email to the participant(s) instructor.


Important Reminders:

  • If the pickup person is not listed as an emergency contact or has a different last name than the child, staff will need to confirm permission before release.

  • To avoid delays, parents should notify on-site staff in advance if someone else is picking up.

  • You must inform us on day one of anyone not authorized to pick up your child. If that person arrives, we will contact the authorities.
